Mother Hen Notice of Privacy Practices (NPP)

This notice describes how medical and mental health information about you and/or your child may be
used and disclosed and how you can access this information. Please review it carefully.
Mother Hen complies with the Health Insurance Portability and Accountability Act (HIPAA) to protect
your privacy and ensure the confidentiality of your health information.

Mother Hen may use electronic communication methods, including email, telehealth platforms, and SMS.
- We take reasonable precautions to protect your information
- However, electronic communication carries some level of risk
- By engaging in these services, you acknowledge and accept these risks

By providing your phone number:
- You consent to receive messages from Mother Hen
- Your personal information will not be shared with third parties for marketing purposes
- Message frequency may vary
- You may reply STOP to opt out at any time
- You may reply HELP for assistance
- Message and data rates may apply
- Complaints
If you believe your privacy rights have been violated, you may file a complaint with:

- The U.S. Department of Health and Human Services (HHS) Office for Civil Rights
You will not be penalized for filing a complaint.
